The Fellowship in RRT aims to equip healthcare workers to explain the basics of science and techniques to their patients to enable them to participate in decision-making to improve long-term outcomes. The fellowship is delivered in the form of 3 wholly online interactive modules. It is main objectives are:
1. Principles of dialysis: during the dialysis sessions, many young physicians are not able to explain what happens to blood when it is pumped into the extracorporeal circuit of a dialysis machine. The fellowship aims to bridge this gap in their ability to appreciate the basics and principles of dialysis.
2. Fundamentals of applied physics: Fellows will be able to explain the applied physics of water treatment and dialysate composition so that they can individualise the treatment specific to a patient.
3. Type of dialysis to choose from: Fellows will have good enough expertise to write a dialysis prescription that is precise, specific, and effective for patients.
4. How to choose a dialyser: Fellows will be able to critically appraise commercial brochures to decide which dialysis membrane is better. They will be able to measure the comparative performance of dialysis equipment themselves to make a measured and informed logical decision to choose the most appropriate approach. Fellows will save themselves from being gullible as there can be pressure from competitors in the market. Fellows will be driven by scientifically proven truth, as is expected from a realistic and professional clinician looking after EKSD patients.
5. How to choose haemodialysis or hemodiafiltration? These are not the same, though there is some parallel between these 2 modalities. The decision is based on what toxins in the blood are retained, how to remove and how to improve the quality of life (living Longer and stronger on the dialysis approach).
6. Three PHs: Fellows will be able to handle any question posed to them in a day-to-day clinical setting. Three PHs are Physics of dialysis, Physiology of toxin productions and Philosophy of your choice in the dialysis prescription.
7. Leadership skills: Each fellow is a leader in dialysis who will educate patients and relatives effectively and run dialysis programs with excellence. The fellows will be able to commission assistants, validate the equipment and complete the commercial tender process.
8. How to improve the outcomes? We understand that 5-year survival for dialysis patients is much lower compared to renal transplantation. The fellows will be able to continuously question how to provide better dialysis for those with a range of comorbidities.
9. Identity with patients and their needs: Five cardinal needs of ESRD patients are: ‘can travel’ (portable dialysis), ‘can eat freely’ (nutrition support), ‘has no pain, ‘has no fear’ of dialysis fatigue, and, lastly, ‘enjoy free days’ between dialyses. These are our goals of therapy to fulfil all patients’ requirements that we will discuss.
10. Pathway of dialysis to transplant: Dialysis Fellowship will complete the courses of transplantation and CKD-MBD by placing the correct pieces of the jigsaw puzzle.
The total number of scheduled learning and teaching hours is 200 hours/module (72 hours of directed student activity and 128 hours of student self-directed learning activity).
The student is expected to spend 16-17 hours studying per topic over the 16 weeks of the module. Additional time is provided to guide the students during the assessment (formal and informal). Our materials will be issued in a cycle every 7 days. Emphasis will be placed upon developing effective clinical reasoning strategies based upon the use of ‘best evidence’ and critical reflection of practice.
